The Congenital Hyperinsulinism International (CHI) Designated Tier 2 HI Center designation recognizes hospitals and clinical programs that provide high-quality, guideline-informed care for individuals with congenital hyperinsulinism (HI) and strengthens access to HI expertise beyond formal CHI Designated Tier 1 Centers of Excellence (COE). Within CHI’s broader designation framework, the COE represents the highest level of recognition, reserved for HI care centers that demonstrate exceptional multidisciplinary clinical care, research leadership, collaboration, and sustained commitment to patients and families. The Tier 2 HI Center is intended to identify experienced centers that can serve as trusted regional or national access points for diagnosis, management, care coordination, family education, and longitudinal follow-up for patients whose needs can be appropriately supported within the center’s scope of services. Tier 2 HI Centers also play an important role in recognizing when collaboration with or referral to a CHI COE is needed for highly complex cases, advanced diagnostic evaluation, or specialized surgical care.
Hospitals and care teams with established HI clinical experience, access to relevant multidisciplinary services, and a commitment to evidence-based, patient- and family-centered care are encouraged to apply. Tier 2 applicants are evaluated across key domains that reflect the core elements of high-quality HI care, including health system affiliation and multidisciplinary support, clinical reach and referral activity, HI specialist expertise, coordinated care and clinical oversight, evidence-based diagnostic and treatment practices, access to genetic testing and counseling, surgical evaluation and referral pathways, continuity of care and transition support, and engagement with research, quality improvement, or evidence-based learning.
The Tier 2 program is an important step in the broader CHI COE framework because it supports a more connected and transparent care network, helps families identify centers with demonstrated expertise in HI, and promotes continued growth in HI care capacity worldwide. By recognizing Tier 2 HI Centers alongside Centers of Excellence, CHI aims to improve access, encourage collaboration, and ensure that more patients can receive knowledgeable, coordinated care as close to home as possible while maintaining clear pathways to COE-level expertise when needed.
